1. Services Provided
Sunshine State Sports Medicine provides athletic training, manual therapy, injury prevention, mobility evaluations, and related wellness services. All services are provided by licensed or certified professionals and are intended to support general health and recovery.
2. Not Medical Advice or Emergency Care
Our services are not a substitute for medical advice, diagnosis, or emergency treatment. If you are experiencing a medical emergency, call 911 immediately. Our staff may provide health-related support, but we do not prescribe medications or offer primary medical care.
3. Eligibility
You must be at least 18 years old or have parental/guardian consent to receive services. You must provide accurate and complete information during intake and scheduling.
4. Client Responsibilities
By using our services, you agree to:

Disclose any medical conditions, allergies, injuries, or concerns that may impact your care

Follow aftercare instructions and trainer guidance

Respect provider boundaries and maintain appropriate conduct at all times

Refrain from recording or distributing any session without prior written consent
We reserve the right to terminate services to any client for inappropriate behavior, failure to disclose relevant information, or violation of these Terms.
5. Cancellations and No-Shows
We require 2 hours notice for appointment cancellations. Late cancellations or no-shows may be subject to a cost of visit cancellation fee.
6. Payment and Refunds
Payment is due at the time of service unless otherwise arranged. We accept hard currency, Square, and Venmo. All sales are final. Refunds are issued only at our discretion in cases of error or service disruption.
7. Limitation of Liability
To the fullest extent permitted by law, Sunshine State Sports Medicine is not liable for:

Any injuries, complications, or adverse reactions resulting from services rendered

Damages arising from your failure to disclose relevant health conditions

Indirect, incidental, or consequential damages from the use or inability to use our website or services
Use of our services is at your own risk, and you agree to release us from liability except in cases of gross negligence or willful misconduct.
